Eagle Optometry provides eye care examinations for some "Managed Care Programs." Patients should ensure that "eye exams" are provided by their Managed Care Plan, not just eyeglasses. Call Eagle Optometry at 365-6868 for an appointment, and provide the receptionist with appropriate insurance information so that eligibility, deductibles, co-pays, and pre-authorization may be obtained if necessary.
*CONTACT LENSES: Patients requesting a contact lens fitting which is not covered by their Managed Care Plan, will only pay the difference between the contact lens exam fee and the regular exam fee, as long as their Managed Care Plan covers a regular exam!
